We are closed during major holidays including New Year’s Day, Memorial Day, Fourth of July, Labor Day, Thanksgiving, Christmas Day. Our office is closed at noon the day proceedings holidays.

We strive to answer all patient calls promptly and during the same business day. If you leave a message after 3 pm, our office may need to contact you the next business day. In some situations, response time may be up to 24 hours due to need to confer with healthcare provider and review information.

If leaving a voicemail, please include your first and last name, date of birth, and preferred pharmacy if calling for a medication or medication refill.
We do not have an after-hours call service for patients. If there is an emergency, you will need to go to your local urgent care center or emergency room for immediate needs.

According to the Health Insurance
Portability and Accountability Act (HIPAA), copies of medical records can only be transferred upon written request. Copies of medical records are provided to patients, physicians, health care providers, insurance companies, attorneys, and Social Security and disability representatives.
To obtain a copy of a medical record, print the Authorization for Release of Information form below, complete it and sign, then fax or mail to the location listed below.

If you are a patient requesting your medical record, the easiest way to do this is through the Patient Portal.

Requests are processed in the order in which they are received. The time it takes to complete your request can vary but is usually completed within one week of request. Federal and state laws govern the release of medical information.
Under Georgia law, you may be charged a fee for administrative and copying services. These fees can change from year to year. Fees are waived for copies provided directly to a health care provider for continuing medical care.
